As reported by Polskie Radio Wrocław, the incident occurred on July 19 in Mojesz (Lwówek Śląski municipality). The Marshal of Lower Silesia and long-serving Bolesławiec County head Cezary Przybylski got a flat tire and lost control of the vehicle, then hit guardrails and the fence of one of the properties. He was sober.
The local government official is bruised and sustained head injuries. His life is not in danger. According to PRW, Przybylski was driving the company car himself because his driver is on leave.
The Marshal was fined PLN 250.

As Mateusz Królak, press officer of the Lwówek police, informed us, the Marshal was not fined for getting a flat tire, but for failing to adjust his speed to the road conditions. The flat tire was only an indirect cause of the incident.
