Individuals and families whose households suffered damage as a result of flooding may receive emergency assistance of up to PLN 8,000, pursuant to the Act of 12 March 2004 on social assistance.
The application must be submitted using Annex No. 1.
The office adds:
IMPORTANT: in accordance with the guidelines of the Minister of the Interior and Administration, assistance is provided upon an application submitted no later than 30 days from the occurrence of an event constituting a natural disaster.
Applications for the above-mentioned assistance are accepted at the headquarters of the Municipal Social Welfare Centre in Kruszyn, 1a Kasztanowa Street, from 8:30 a.m. to 3:00 p.m., by social workers serving the localities of Gmina Bolesławiec. Applicants must bring an identity card or another document confirming their identity.
Applications may also be submitted electronically via the Electronic Platform of Public Administration Services (ePUAP) by sending the above-mentioned application to the mailbox address: ePUAP:/GOPSBolesławiec/skrytka ESP. The application must bear a qualified electronic signature, a trusted signature or a personal signature.
