Bolesławiec Healthcare Complex:
In connection with the announcement by the Ministry of Health of the National Vaccination Programme on 15 December this year (published on the website www.mz.gov.pl), we inform you that Zespół Opieki Zdrowotnej w Bolesławcu, designated as a “hub” hospital, will vaccinate medical and non-medical staff against COVID-19, including staff from other institutions such as hospitals, clinics and pharmacies. We are accepting submissions from medical entities until 19 December 2020.
Entities interested in having their employees vaccinated at Zespół Opieki Zdrowotnej w Bolesławcu are asked to send their submissions to: szczepieniacovid@zozbol.eu.
ZOZ adds:
The submission should include the entity’s name and address, REGON number, contact person’s details (first name, surname, email address and telephone number), as well as the first names and surnames and PESEL numbers of the employees being registered, together with a scan of consent to the processing of personal data (template below).
We are accepting submissions from medical entities until 19 December 2020. The submission should be sent as an encrypted attachment to the email address szczepieniacovid@zozbol.eu.
