  • in person or through an authorised representative at the Customer Service Office of Urząd Miasta Bolesławiec, pl. Piłsudskiego 1a (office building), during the following hours:
    • Monday, from 8:00–17:00,
    • Tuesday to Friday, from 8:00–15:00.

At the same time, we remind you that applications may also be submitted:

  • by post to: Miejski Ośrodek Pomocy Społecznej w Bolesławcu
    ul. Cicha 7, 59-700 Bolesławiec,
  • electronically via the ePUAP platform: /mopsboleslawiec/skrytka (the application must bear a qualified electronic signature, a trusted signature or a personal signature in accordance with the Act of 17 February 2005 on the computerisation of the activities of entities performing public tasks).
