In 2017, the program “Screening Tests – Prevention of Breast Diseases in Women” has two providers: Centrum Medyczne Stomadent in Bolesławiec and Centrum Specjalistyczne Biomed in Bolesławiec. As part of the program, mammograms with a report and breast ultrasounds with a report are performed. The preventive diagnostic examinations are intended for women aged 40 to 50 and women over 69 who live within the city of Bolesławiec. The program does not cover people who used diagnostic examinations financed by Gmina Miejska Bolesławiec in 2016 as part of breast-disease prevention for women, except where repeating the examination is recommended by a doctor.
The examinations are performed upon presentation of an identity document confirming the patient’s age, sex, PESEL number and place of residence, as well as, where applicable, a doctor’s recommendation.
The examinations are performed at Centrum Medyczne Stomadent in Bolesławiec, at 21 Dolne Młyny Street. Patients arrange the date of each examination in person or by telephone at 75 734 68 68, Monday to Friday, from 8:00 a.m. to 8:00 p.m.
Another location where the examinations are performed is Centrum Specjalistyczne Biomed in Bolesławiec, at 14 Komuny Paryskiej Street. Patients arrange the date of each examination in person or by telephone at 501 084 798 or 75 647 34 80, Monday to Friday, from 10:00 a.m. to 5:30 p.m.
“Screening Tests – Prevention of Breast Diseases in Women” supplements the program financed by the National Health Fund for women aged 50 to 69 (breast mammography).
The “Caries Prevention in Children” program is intended for second-grade primary-school pupils. The provider is Chodyra Clinic Dominik Chodyra in Bolesławiec. As part of the program, an examination of the oral cavity is performed, including an assessment of dental health, with particular attention to the presence of carious lesions and malocclusion (preparation of a dental chart), for children in second grade attending Bolesławiec public primary schools. Parents also receive guidance on further care for the child. The examinations are performed on the basis of lists prepared and approved by school principals, after the child’s parent or guardian has registered at Chodyra Clinic in Bolesławiec. The examinations take place at Chodyra Clinic. Clinic of Implantology and Orthodontics at 1/1A Garncarska Street in Bolesławiec.
