An information meeting on the “Life Envelope” initiative was held at Bolesławiec City Hall, with representatives of, among others, ZOZ, uniformed services, MOPS and pastors from Bolesławiec parishes. The “Life Envelope” initiative was created in response to public expectations that seniors should be able to feel safe in any situation, especially when they cannot count on help from their nearest and dearest. It often happens that emergency services enter the home of an elderly person living alone who, because of illness or loss of consciousness, cannot provide the information necessary to carry out a rescue operation. When there is no time or opportunity to conduct an appropriate interview, gathering all the basic information about the household member’s health in one place is extremely helpful and makes the work of rescue and medical services easier.

The “Life Envelope” consists of placing the most important information about a patient’s health, medications they take, drug allergies, contacts for their nearest and dearest, and personal details, including their PESEL number, in specially prepared plastic envelopes. The questionnaire will also include the name of the attending physician. A package containing this information should be kept in a visible place in the refrigerator, additionally marked with a sticker reading: “The Life Envelope is here.” The envelope will include instructions for completing the information card.

Participation in the “Life Envelope” program is entirely voluntary and free of charge. Every person participating in the “Life Envelope” program will be required to update the information about their health annually.

The first “Life Envelopes” will appear in Bolesławiec on November 23, 2016, during the inauguration of Bolesławiec Senior Day.
