How can you obtain a certificate for a child?

To obtain such a certificate, you must complete an application for its issuance. Other documents that should be prepared include all opinions from psychologists and test results, such as magnetic resonance imaging scans, X-rays or ultrasounds in which abnormalities were detected.

Your application is assessed by an adjudicating panel consisting of a doctor specializing in the relevant disability and another person, such as a social worker, psychologist or careers adviser. During the assessment, the panel considers the period for which assistance is required and the activities in which your child is not independent.
