The aim of the Program is to introduce personal assistance services as a form of broadly accessible support in performing everyday activities and functioning in social life. The Program is addressed to:

  1. children under 16 with a disability certificate that includes the indications in points 7 and 8 of the disability certificate—requiring permanent or long-term care or assistance from another person due to significantly limited ability to live independently, and requiring the child’s caregiver to participate continuously in the child’s treatment, rehabilitation and education on a daily basis, as well as
  2. people with disabilities who hold a certificate of a severe or moderate degree of disability, or who are treated as equivalent to holders of the certificates listed above.

Personal assistance services involve an assistant supporting a person with a disability in various areas of life, including:

  • supporting the participant with self-care activities, including maintaining personal hygiene;
  • supporting the participant in running a household and fulfilling family roles;
  • supporting the participant in travelling outside their place of residence;
  • supporting the participant in undertaking life activities and communicating with their surroundings.

Applications for the Program will be accepted from 19 February 2024 to 23 February 2024. Interested persons are asked to submit applications at the Municipal Social Welfare Centre in Bolesławiec, 7 Cicha Street, Monday to Friday, from 7:00 a.m. to 3:00 p.m., or electronically via the e-PUAP platform (the application must bear a qualified electronic signature or be authenticated using a trusted profile).

At the same time, we inform you that declaring the intention to participate in the program does not constitute qualification for the Program. Applicants will be notified of the recruitment results by telephone and in writing.

The criteria used to qualify for the Program include, above all:

  • the degree and type of disability
  • family situation
  • age
  • participation in previous editions of the program
  • order of applications.
