The “Innovation and Implementation Project in the Field of Functional Assessment” involves researching and developing model solutions for locally provided, intersectoral support for children, students and families, using the International Classification of Functioning, Disability and Health – ICF.

The main aim of the project is to improve the quality and effectiveness of coordinated, intersectoral support provided to children, students and their families, as well as to the environment in which they function at the local level (the county).

To achieve this aim, during the project the institutional resources, forms of community support and programmes implemented by local-government units and non-governmental organisations in the health, education and social assistance sectors were analysed. Efforts were made to integrate activities focused on the child, student and family, understood as combining into a single support instrument the many current organisational forms supporting children’s development until they fulfil the compulsory education requirement and during its fulfilment.

The team working on the project sought to implement intersectoral cooperation and coordination of activities, particularly in the areas of healthcare, family assistance and social assistance within the child’s environment. Throughout the process, the personalisation of support was important, achieved by adapting the type and scope of assistance to the family’s actual needs and reducing the procedural burden associated with accessing help.

The team also considered how to use local resources effectively and, consequently, how to improve the quality of assistance provided through consistent objectives and synchronised activities.

The project also aims to support change processes in local communities, with particular emphasis on the roles of coordinating units (JST), supporting units (PPP, first- and second-level referral facilities in healthcare) and cooperating entities (preschools, schools, educational institutions and non-governmental organisations). The amount of funding for project activities in Powiat Bolesławiecki is PLN 224,674. The project is scheduled to conclude in June 2023.
