How can you register for vaccination?
You can register in several ways:
- Registration at ZOZ Bolesławiec by telephone at 75 738 02 03. By telephone, you will receive information about the location, date and time of your vaccination.
- Registration via the 24-hour, toll-free helpline at 989. Your PESEL number is sufficient to register. A mobile phone number is not required, but if you provide one, you will receive an SMS confirming your vaccination appointment. During registration, you will choose the exact date and location of your vaccination. Then—if you provided a telephone number—you will receive an SMS confirming your vaccination appointment.
- Through e-Registration available at pacjent.gov.pl and a trusted profile. The system will offer you five available appointments at vaccination points located near your address. If none of the proposed appointments suits you, or if you want to be vaccinated at another point in another town, this will be possible. Simply use the available search engine and select a convenient date and location. Immediately after making the reservation, you will receive an SMS notification, and we will remind you about the vaccination again one day before the scheduled appointment.
- By contacting your chosen vaccination point. If you want to be vaccinated at a specific vaccination point, contact it. Map and contact details of vaccination points.
- By sending an SMS to 664 908 556 or 880 333 333 with the text: SzczepimySie. By sending an SMS to one of the numbers listed above (both numbers connect to the same system), you will be connected to a system that will guide you step by step. First, you will be asked to provide your PESEL number, followed by your postal code. The system will offer you the nearest available vaccination appointment at a point close to your place of residence. If that appointment is not convenient for you, you will be able to choose other dates. After registering, you will receive an SMS reminding you of the appointment time and location one day before vaccination.
