People applying for disability assessments—adults, as well as parents and guardians applying on behalf of their children—are being served by a medical panel working with its full membership. The difference is that it is not possible to appear in person before the panel. Specialists issue decisions solely on the basis of medical documentation, which should be delivered to the submission box at the district office headquarters at 12 Armii Krajowej Street or sent by mail.

The specialists currently working are: a surgeon (also issuing decisions in orthopedic conditions), an otolaryngologist, internists, neurologists, pediatricians, including a pediatric neurologist, and a psychiatrist.

At the same time, we would like to inform you that on 31 March this year, the Act—the anti-crisis shield—entered into force. It extends the validity of disability assessments and parking cards until the 60th day from the date on which the state of epidemic emergency or the state of epidemic is revoked, but no longer than until a new disability assessment or disability-degree assessment is issued.

This applies to disability assessments and disability-degree assessments issued for a fixed period, whose validity:

  1. expired within 90 days (i.e. from 9 December 2019) before the Act—the shield—entered into force (i.e. by 8 March 2020), provided that another application for an assessment was submitted during that period
  2. expires during the period beginning on the date the Act—the shield—entered into force (i.e. from 8 March 2020), without the need to submit an application for an assessment.

Because disability assessments and disability-degree assessments remain valid for the period referred to above, parking cards issued on the basis of those assessments remain valid until the 60th day from the date on which the state of epidemic emergency or the state of epidemic is revoked, but no longer than until a new disability assessment or disability-degree assessment is issued.

The extension takes effect by operation of law, on the basis of the provisions of the Act.

In case of doubts concerning the extension of the validity of assessments or difficulties with correctly completing the documents, team staff will provide information by telephone and help complete the forms efficiently. Tel. 75 612 17 04, 75 612 17 05.
