The Card promotes the large-family model and shapes its positive image. It introduces a discount system, and its holders can use the offers of Program Partners—municipal institutions and organizational units operating in the fields of culture, sport and recreation, as well as businesses and other entities—on preferential terms. The list of institutions declaring their participation in the Program will grow.

  1. Applications for the Card may be submitted from 1 October 2015 throughout the year at Bolesławiec City Hall in the town hall, at the Customer Service Office, from 9:00 a.m. to 1:00 p.m. on working days. The application form can be obtained from the Customer Service Office and downloaded from the website: bkdr.boleslawiec.eu.
  2. The application, containing complete details of all eligible family members, must be completed legibly in block letters and signed by all adult family members.
  3. A condition for receiving the Card is consent to the processing of personal data for the purposes of implementing the Program, in accordance with the Act of 29 August 1997 on the protection of personal data.
  4. The application must include photocopies of, and the originals of the following documents must be presented for inspection:
    1. identity documents of all adult family members;
    2. school ID cards, in the case of school-age children and young people;
    3. abridged birth certificates, in the case of children who have not yet begun compulsory schooling;
    4. a certificate, student ID card or other document confirming continued education, in the case of young people over 18 who are studying;
    5. a court order, in the case of children and young people residing in foster care;
    6. a decision confirming a significant or moderate degree of disability, in the case of an adult family member with a disability.
  5. The applicant may be requested to provide explanations or supplement the application.
  6. A personalized Card, bearing an expiry date and number, is issued free of charge within 14 days of submitting a correctly completed application, individually for each Eligible Person.
  7. The Card is valid for one year and entitles its holder to use the benefits available under it upon presentation of an identity document, or, in the case of a child without their own document, together with the guardian’s Card and a document confirming the guardian’s identity.
  8. An adult family member confirms receipt of the Cards for family members by presenting an identity document and providing a legible handwritten signature.
  9. In the event of loss or damage to the Card, the Eligible Person must immediately submit an appropriate statement at City Hall in the town hall, at the Customer Service Office, from 9:00 a.m. to 1:00 p.m. on the Office’s working days.
  10. Issuing a duplicate Card requires submitting a new application.
  11. A fee of PLN 15 is charged for issuing a duplicate Card. It must be paid at the cashier’s office into the bank account of the Municipal Social Welfare Centre in Bolesławiec: 40 1020 5226 0000 6102 0507 9878.
  12. Proof of payment must be attached to the application for a duplicate Card.
  13. Eligible Persons may not lend the Card to third parties, under penalty of withdrawal of the granted entitlement.
  14. If the Card is used by an unauthorized person, the Program Partner has the right to retain the Card and hand it over to Bolesławiec City Hall in the town hall, at the Customer Service Office, from 9:00 a.m. to 1:00 p.m. on the Office’s working days.
  15. The list of Program Partners, the services and goods offered, and the amounts of discounts, exemptions and rebates available to Eligible Persons is published on the website www.boleslawiec.eu under the “Bolesławiec Large Family Card” tab.

  1. All members of Large Families residing in the Bolesławiec urban municipality are entitled to use the discounts and benefits offered by Program Partners where a parent or the parent’s spouse supports at least three children, regardless of income. In particular, Eligible Persons include:
    1. a guardian—a parent, including a foster parent or a person running a family children’s home, a parent’s spouse, or a legal guardian;
    2. a child—a biological child, a spouse’s child, a child under legal custody, an adopted child or a child for whom adoption proceedings are pending, up to 18 years of age; a studying child up to 25 years of age; and a child with a significant or moderate degree of disability, regardless of age, provided that the child is not married.

The main objectives of the Program are:

  1. promoting the large-family model and shaping its positive image as an element of the social policy of the Bolesławiec urban municipality;
  2. building a favorable local climate conducive to making decisions beneficial to family development;
  3. supporting large families by enabling them to use, on preferential terms, the offers of municipal institutions and organizational units operating in the fields of culture, sport and recreation, as well as discounts and preferential services granted by businesses and other entities;
  4. supporting the budgets of families with many children burdened by the costs of maintenance;
  5. improving economic conditions in local services and goods production through discounted terms of use for large families (this should improve the condition of Program Partners—local-government institutions and businesses participating in the “Bolesławiec Large Family Card” program).

The scope of entitlements resulting from the Program and available to Eligible Persons includes a system of discounts, exemptions and preferences covered by the Card catalogue and offered by the Program Implementers.

The Program Implementers are:

  1. Bolesławiec City Hall;
  2. Municipal Social Welfare Centre in Bolesławiec;
  3. Program Partners—municipal organizational units and other interested entities that offer discounts and preferences on the goods and services included in their offer and join the Program.

Program Implementers place information about honoring the Card, with the wording “Bolesławiec Friend of Large Families,” on their websites, in visible locations at their premises and on their websites.

The document entitling its holder to use the discounts and preferences offered by Program Implementers is the “Bolesławiec Large Family Card.”

The rules for issuing and using the Card are set out in the Regulations of the “Bolesławiec Large Family Card” Program.
