Due to high interest, 7 and 25 November the mobile ZIP (Integrated Patient Information) stand will once again be available at the Bolesławiec Tax Office (from 9:30 a.m. to 3:00 p.m. in the Customer Service Room on the ground floor).
It will be possible to register with the National Health Fund system and receive a login and password. This information allows everyone to view their medical history from 2008 onward and the list of prescribed reimbursed medicines. It also allows users to check their current insurance status in e-WUŚ.
Adults interested in accessing ZIP should appear in person with proof of identity (an identity card or passport). More information on the National Health Fund website.
