The theft or loss of an identity card or another identity document is a very serious problem that can lead to a range of difficult-to-explain situations involving the use of our data for criminal purposes. What should we do when we lose our documents? Do not wait—report the loss as soon as possible to your bank and to the institution that issued the document.
What should you do if you lose your documents?
LOST DOCUMENTS SHOULD FIRST BE INVALIDATED AT A BANK—there are several thousand branches throughout the country (in many of them, you do not even need to have a bank account). Below, we present the next steps to take:
1. Report the document in the DOKUMENTY ZASTRZEŻONE System—at your own bank or at any bank that accepts reports from people who are not its customers as well (see the list of banks).
- It is enough to report the document at just one bank; the data will automatically be forwarded to all other participants in the DZ System.
- It is best to do this in person at a bank branch. Some banks also accept reports by telephone at 828 828 828 (only from their own customers, after remote identity verification).
- You can use an account on www.bik.pl (only if the person reporting the document had previously opened an account there using the data from the lost document).
2. Notify the Police—if the documents were lost as a result of a crime.
3. Notify the nearest municipal authority or consular office and obtain a new document,

We emphasize that all banks in Poland and a number of other companies and institutions participate in the DOKUMENTY ZASTRZEŻONE System. However, the full list of participants is kept secret for the security of the entire System.
