Registering your interest in vaccination through the online form is not registration for a vaccination appointment. You will be able to register when vaccination dates for your age group are announced and you receive an e-referral.

How can you register for vaccination?

Go to the website: https://szczepimysie.pacjent.gov.pl/

  1. Fill in the simple form (provide your first and last name, PESEL number, email address, postal code and, optionally, your telephone number).
  2. You will receive a message at the email address provided in the form, asking you to confirm your registration.
  3. Open your email and click the link – your form will be approved and saved in the system.
  4. When registration for vaccinations for your group begins, you will receive an email informing you that an e-referral has been issued for you. You will then be able to register for a specific date.
